All that I had was my Tx DL and an insurance card with the vin# from both of my machines. Dont think you need insurance, just the vin#.

There was no form to fill out!

Just take the VIN , insurance, title, DL and ask to get off road license plates?

Yes I only had an insurance card with the vin#, and my DL. No forms to fill out just pay the fee. $15 each machine. Not sure if all counties have the plates in stock, they just became available.

Yea I lived in Arlington for about 20 years doing field service in Texas, Louisiana, and Oklahoma. Drove a lot of back roads.

There is no form required. Simply visit your local county tax office and request a plate. Please note huh that this is not ‘registration’. No registration record will be created nor will a sticker be issued.

Not all counties had the plates in stock. Im sure some still dont have them.

"Operation may be authorized only on roads with a speed limit of 35 mph or less. If operated on the roads at a speed of 25 mph or less, an ATV, ROV, UTV and Sand Rail is required to display a slow-moving vehicle emblem "

Our FM roads have a 50 MPH speed limit. Does the above seem to allow operatiom on roads with a 35 MPH speed limit, this outlawing our FM roads. Hope not.
