The holes typically wear faster if you change from stock arms to aftermarket on the 16-21 models.
I have a 16 with roughly 3000 miles on it. I am starting to get a wandering feeling when driving it at moderate speeds. I have some shop time scheduled later this season to inspect my machine to see where the wander is coming from. Right now it is used as a glorified golf cart at camp. No real danger going under 10 mph.

Too many variables to give a concrete answer. The single bolt upper is a problem that has been addressed by Honda for the 22 machines. They went from a single 10mm bolt, to a dual 12mm bolt. It is also common for the welds to fail holding the tabs to the frame. I am working with my laser fab guy to get some different bracket options designed and cut. That is longer term though.

I like using 2x2x.125 wall square tubing and you cut a 45° piece off for a nice 90° gusset. Mark the bolt hole in the gusset tab and then weld the washer on before welding the gusset to the frame. Also make sure there's a vertical weld on both a-arm tabs to the frame. Seems Honda missed that weld or didn't go the full height behind the a-arm pivotQuick fix for chores but picked up the steel to make gussets.
